1. The schematic Trash80 made is confusing. I recommend looking at this video since the schematic for IN is the same but it explains it better.
http://www.notesandvolts.com/2015/02/mi … input.html
http://www.notesandvolts.com/2015/03/mi … utput.html

2. Leave pins 1 and 4 floating. In Trash80's schematic 7 is floating as well, but nitro2k01 recommends connecting 7 to ground through a 4.7k resistor.
http://blog.gg8.se/wordpress/2009/07/26 … -glitches/

3. Leave all the extra midi pins floating. If you want to ground them you probably could but it's unnecessary.

Shameless plug: I'm selling kits here for pretty cheap that include a PCB so it's easier to hook everything up. If you're interested hit me up.

Do pins 1, 4, and 7 on the 6N138 need to be connected to ground or can they just be left floating?

Looking at the 6N138 datasheet, pins 1 and 4 are NC, not connected to anything, so can be left unconnected. Pin 7 is connected to the base of the 6N138's output transistor and should be left open. If you ground pin 7, the Arduinoboy won't work.
