The headphones-out & mic-in aren't mods it came like that. The 1/4 inch output is a mod. But yea, it looks very Yamaha except it has no brand/manufacturer information on it anywhere. Only a "Made in China" on the back.
I don't know who the bender is otherwise I'd refer you to them! :0 Sorry!
