Case from Radio Shack, I think it's the 5x8 inch case.  The holes were made with a Step Bit.  Can't say enough good things about Step Bits.

finally tested it. thanks to Alley Beach (formerly pr0n) for building it. Really great guy to have done it for me. I will post pictures when I find my cameras battery.
