If it has a standard gameport adapter you could probably solder directly to a arduionobo teensy:

Not gonna debate the ethics of sharing his music as a shadow account, but id defo consider changing the license on all those tracks to the more restrictive all rights reserved, before the tracks end up in some dumb student game.

With regards to sync, piggy responds to keypresses in the background using a joystick, so if you assigned piggy to have the same keybindings for both apps, and bgb was in focus, they would both start at the same time and should more or less stay in sync. At least it did 8 years ago when I first tested this. Im pretty sure this DOESNT work for the keyboard though.

how to make it work on 64bit debian?
I spent some time messing with this, and you can install the 32bit architecture, but then I was in dependency hell.
In contrast installing a 32bit ubuntu only required me to instal SDL and I was in business!
